Why Saint Lucia

Saint Lucia is the one territory in this network with a Clinical Trials Act on the statute book, designating the Ministry of Health as National Competent Authority with the power to issue trial authorisations. It also has the tightest confidence intervals on its prevalence estimates in the Eastern Caribbean — a sign that real national survey data, not just modelling, sits underneath the numbers.

The disease burden

Saint Lucia's estimates are unusually well constrained — obesity 33.7% [30.6–36.8] against intervals ten points wide elsewhere in the Eastern Caribbean. For feasibility modelling that matters: you can size a screening population here with more confidence than almost anywhere else in the region.

• Owen King European Union Hospital, Castries — 120 beds including a six-bed ICU, theatres, MRI and laboratory; opened fully March 2020, EUR 50.85m EU-funded

Research heritage

The St Lucia Project (1965–1981), funded by the Rockefeller Foundation with the Government of Saint Lucia through a dedicated Research and Control Department of the Ministry of Health — the first large-scale concurrent comparison of drug-based against environmental control of Schistosoma mansoni, using the island's geographically isolated valleys as parallel study arms. A 2020 national survey found evidence that transmission had been interrupted. This is heritage rather than current capability: the infrastructure was decommissioned decades ago. But it establishes that large controlled studies have been run successfully on this island before.

Regulatory & ethics

• Clinical Trials Act — section 7 designates the Ministry of Health as National Competent Authority, with power to issue authorisations, monitor and enforce
• Section 11 requires an Ethics Committee opinion; sections 13 and 15 govern application for and grant of authorisation
• Section 6 provides for a statutory Ethics Committee of 7–11 members appointed by the Minister with Cabinet approval

The honest position: The Act exists, but there is no published evidence that an Ethics Committee has been appointed and is sitting, no published authorisation guidance, and no fee or timeline schedule. A sponsor could file into a statutory process that has never been exercised. Note also that PAHO's Pharmaceutical Country Profile still states no authorisation requirement exists — that profile appears not to reflect the Act. We establish the live position with the Ministry before committing a protocol, and we tell you what we find.

Data quality: Strong relative to peers — the tightest confidence intervals in the Eastern Caribbean and the only territory with clinical trial primary legislation.
