Why Montserrat
We are including Montserrat for completeness of network coverage, not as a recruitment proposition. The territory has roughly 3,500 adults in total, a 30-bed hospital operating out of eight buildings of a former primary school, and no published NCD epidemiology of any kind. Presenting it as a clinical trial site would not survive contact with a feasibility team.
The disease burden
WHO publishes no obesity, overweight, diabetes, hypertension or NCD mortality estimates for Montserrat — the territory falls below the population threshold for Global Health Estimates. PAHO's country profile carries demographic, maternal and child health, TB/HIV and COVID indicators only. There is no epidemiological basis on which to size a study population.
Sites and infrastructure
- Glendon Hospital, St John's — 30 beds, 24-hour emergency department; laboratory limited to simple haematology, biochemistry and blood banking with no microbiology; portable X-ray only
- A replacement 57,000 sq ft, 25-bed national hospital is under construction with UK FCDO support

Research heritage
Genuine but environmental rather than clinical — the Soufrière Hills eruption generated a body of respiratory health research on volcanic ash exposure in children, catalogued by UKHSA. No cohort, no trial, no health research institution.
